Following a comprehensive assessment of the patient’s needs and status, Supportive Home Health Care Corporation supervisors will determine which clinical professionals are best suited to accomplish the goals set for the particular patient under the supervision of the attending physician.
Once treatment is initiated, personnel performance is consistently supervised, monitored, and evaluated to ensure that the most appropriate care is rendered.

Our quality assurance is of paramount importance in all of Supportive Home Health Care Corporation assignments. We have developed, and maintained a formal and comprehensive set of policies, procedures and guidelines that constitute our “QI” program. This program is administrated by the Agency administrator.

> Quality Improvement Plan
The quality assurance activity at Supportive Home Health Care Corporation shall be designed to objectively and systematically monitor and evaluate the quality and appropriateness of patient care. It will also determine if services provided are in Compliance with Corporate, State, Federal, and Private payer regulations. This will be accomplished by utilizing a variety of techniques which will identify key areas where improvements are necessary, patient care, coordination between patient services and the reduction or resolution of identified or suspected problems.
> Scope of Service
Supportive Home Health Care Corporation provides individual and family-focused full-spectrum services to the patient in his/her place of residence.
Those services are multi-disciplinary in nature and involve the knowledge and skills of various professionals from many fields, including Nursing, Rehabilitation Therapies (Occupational Physical, Speech, and Enterostomal), Medical Social Services, Diet Counseling and Home Health Aide.
Admission to those services is open to patient’s aged 18 and over and all diagnostic entities. The individual in need of those services must be under the care of a physician, must be home bound, and must require part-time or intermittent skilled nursing care Highly therapeutic modes are utilized and performed according to the physician’s orders. Those include infusion therapy, various types of parental nutritional support, laboratory services, etc…
Nursing services are available seven days a week, 24 hours a day, With on-call consultation available 24 hours a day.
