History and Background
Supportive Home Health Care Corporation was established under the articles of incorporation and executed on the 5th day of April 2005 to operate as Supportive Home Health Care Corporation. The principal office for the transaction of business of the corporation is located at Supportive Home Health Care, 17141 Ventura Blvd, Suite 206 Encino, CA 91316. The governing body of the corporation, the board of directors, assumes overall legal authority and responsibility for the operation of this agency as defined by Article 1 Sections 74600 and 7463 of the code of regulations. The board ensures that the administrative and supervisory functions are not delegated to another agency or organization and that all services, professional and otherwise, are provided, monitored, and controlled through this agency either directly and/or through contracted services.
Supportive Home Health Care is a proprietary home health care service, licensed in the State of California and Medicare/MediCal certified. Supportive Home Health Care Corporation was established to offer a comprehensive healthcare program of the highest quality to the patients in their place of residence. The agency is committed to the provision of home health services that will assist the client in recovery or stability from illness or disability.
Supportive Home Health Care Corporation will directly provide the services of skilled nursing, home health aides, Physical, Occupational, Speech, as well as Medical Social Services, and Dietary Counseling Services. The geographical coverage of the agency includes all of Los Angeles County.
The services offered by the agency are performed under part time or intermittent basis to home bound patient’s following a written plan of treatment which is evaluated and updated by communication between the Agency’s staff and the Attending Physician at least every 60 days. The Agency has a full time administrator and a full time Director of Nursing who is professionally licensed and qualified; both are responsible to the Board of Directors for the Agency’s total functions. A Medical Advisor, who is a physician duly-licensed under the provision of the California Business and Professional Code, is available in an advisory capacity to the Administrator/Director of Nursing in planning for the coordinated care of the patient.
