You, your family, or your doctor can call our agency when home care services are needed. We will coordinate your care with your physician who supervises the services you receive.

As with all referrals, we will:
- Obtain physician approval and all necessary authorizations.
- Schedule an initial assessment to determine the patient’s health needs.
- Coordinate the patient’s plan of care with the physician.
- Calendar visiting care for the patient.
- Report the patient’s progress to the physician.
- Survey the patient to gather input on the care provided.
Supportive Home Health Care Corporation recognizes that the physician’s time is valuable; therefore, we will try to limit our contact to the following:
- Orders for treatment/discharge
- 30/60 days summary and discharge reports
- Significant changes in the patient’s conditions
